![]() ![]() Visual acuity (best with correction or pinhole) was converted from Snellen acuity to logMAR to allow statistical analysis. Patients whose operations involved the use of silicone oil were not considered to be primary or ultimate successes unless oil was subsequently removed without redetachment. Ultimate anatomical success included primary anatomical success as well as those patients for whom a reattached retina was achieved with subsequent surgery. Primary anatomical success was defined as a reattached retina after one procedure and remaining attached to final follow-up. The operations were carried out between 13/11/98 and 10/2/01 and follow-up was 4 months to 2.5 years. Trainees performed 93 of the 185 operations. The series was an unbiased selection of cases under the care of two consultants (THW: n=110 and DAHL: n=75) in a vitreoretinal unit of a teaching hospital. Macular detachment was defined as detachment involving the fovea with any resulting loss of central Snellen visual acuity. 4 This study of the surgical management of macula-off retinal detachments determines anatomical and visual outcomes and attempts to determine which preoperative factors influenced these outcomes in the setting of a modern vitreoretinal unit.Ī retrospective casenote review was carried out of 185 patients presenting with primary macula-off retinal detachment. In recent years, a shift towards the increasing use of primary internal pars plana vitrectomy away from these conventional buckling techniques has been seen. The reasons for poor visual acuity cannot always be determined and previous studies relate mainly to the outcomes for patients who have undergone ‘conventional’ external scleral buckling surgery with or without external drainage. 3 However, for retinal detachments with macular detachment primary anatomical success does not necessarily achieve a good visual outcome and the functional results vary greatly. 1, 2 Failure to achieve a primary anatomical success (retinal reattachment with one operation) reduces the chance of a good visual outcome. The primary anatomical success rate of surgery for retinal detachment can be high and should be in the range of 80–90%. Preoperative PVR, number of breaks and duration of detachment also affect outcomes. Preoperative visual acuity is the most important factor predicting primary and final anatomical success as well as visual outcome. The median logMAR acuity increment of 0.78 far exceeds that seen in cataract surgery. In all, 76% of macula-off detachments may be repaired with one operation and 44% of patients regain at least 6/12 Snellen. For prediction of visual outcome (in patients with no ocular comorbidity) only preoperative logMAR visual acuity achieved statistical significance ( P=0.001) at the P=0.05 level. In all, 44% of patients regained 6/12 Snellen or better with a median improvement of 0.78 logMAR. Preop logMAR visual acuity and duration of macular detachment were the statistically significant factors predicting ultimate success. Statistically significant factors predicting primary anatomical success using a multiple variable model were preoperative logMAR visual acuity, preoperative PVR and number of breaks. Patients with oil in at final follow-up were considered to be anatomical failures. Primary and ultimate anatomical success were achieved in 76 and 84% of cases. Logistic regression analysis was used to determine (1) the effect of these factors on visual outcome and (2) their effect on primary and final anatomical success. Demographic and ocular characteristics were determined. MethodsĪ retrospective casenote review of 185 patients presenting with macula-off retinal detachment was performed. ![]() ![]() "We know large constrictors can be dangerous to people. "So it's possible that the snake constricted Brandon because of being startled or shifting to predator mode," he said. He adds that there are two common reasons pet snakes constrict their owners-they may constrict out of fear, or when they smell prey, and their predator instincts are triggered. "They may become familiar with their owners or keepers, particularly by their smells, and may rest on them for warmth or just climb on them for activity whenever they are being handled," he says. Moon agreed that snakes don’t show affection in the same way the word is used to describe cats or dogs. "If they're in a tree and suddenly a branch moves or breaks, they're going to try to grab hold with the part touching the tree." "They attempt to grab onto things because they feel nothing is supporting them," he says. Snakes, and other reptiles in general, aren't known to typically show these behaviors.īoback offers another theory: It's possible, he says, that Tiny felt she was going to fall. While the coroner hypothesized that Tiny was showing some sort of affection, Boback says that's likely a mischaracterization.Īffection, as we refer to it, generally refers to physical contact to express some sort of bond. "The constriction pressure would have caused asphyxiation, compounded by the faster effects of disrupted blood flow and possibly disrupted neural function," says Moon. He adds that constriction around the torso can put pressure on internal organs and damage blood vessels like the hemorrhages found in Brandon's eyes. "Disruption of blood flow and brain function can cause unconsciousness and become lethal much faster than suffocation alone," says Brad Moon, a National Geographic explorer who's authored several papers on how snakes kill by constriction. "Your brain is such an oxygen hog, that if you deprive it of oxygen there's a mechanism in your body that just sort of shuts off," he says.Įxactly how long it takes for constriction to cause death isn't quite known, but a person can lose consciousness from this in seconds. With eight feet of muscle, constrictors can constrict blood flow to the brain in a matter of minutes. It suggests they kill their prey by cardiac arrest." But, he adds, "The scientific evidence to date would suggest snakes don’t kill by asphyxiation. "Is it impossible? No," says snake expert and Dickinson College professor Scott Boback. They don’t, despite popular misconceptions, typically kill by suffocating their prey. These pythons are a type of snake called a constrictor, which kills by cutting off blood circulation. In 2013, a snake of the same species killed two boys in Canada. It's the largest snake in Africa and can grow to be 20 feet long. Tiny is an African rock python, a species of snake with an especially formidable reputation. ![]() "He made clear he did not believe the snake had been aggressive towards its owner, but the most likely scenario was that the reptile had been coiling around him in an affectionate way," the Guardian reported. Still, the coroner concluded that the python was involved in Brandon's death. ![]() This lung and eye damage could have been caused by pressure applied to the chest or neck, but Brandon was found without physical markings in either area. The pathologist indicated these were signs Brandon died by asphyxiation. Ĭontrol and protection are difficulties to microgrids, as all ancillary services for system stabilization must be generated within the microgrid and low short-circuit levels can be challenging for selective operation of the protection systems. ![]() A stand-alone microgrid can effectively integrate various sources of distributed generation (DG), especially renewable energy sources (RES). They offer an option for rural electrification in remote areas and on smaller geographical islands. Ī stand-alone microgrid has its own sources of electricity, supplemented with an energy storage system. This kind of grids are called ' islandable microgrids'. In this way, they improve the security of supply within the microgrid cell, and can supply emergency power, changing between island and connected modes. Ī grid-connected microgrid normally operates connected to and synchronous with the traditional wide area synchronous grid (macrogrid), but is able to disconnect from the interconnected grid and to function autonomously in "island mode" as technical or economic conditions dictate. A ' Stand-alone microgrid' or ' isolated microgrid' only operates off-the-grid and cannot be connected to a wider electric power system. It is able to operate in grid-connected and in island mode.
